Medicare IRMAA Calculator (2026)
If your income is higher, Medicare charges you more for Part B and Part D. This extra is called IRMAA, and it is based on your tax return from two years earlier. Going $1 over a line puts you in the higher tier for the whole year. Enter your income to see where you land.
MAGI = adjusted gross income + tax-exempt interest. 2026 premiums use 2024 income; 2027 premiums will use 2025 income.
2026 IRMAA brackets
| Single | Married filing jointly | Part B per month | Part D extra per month |
|---|---|---|---|
| Up to $109,000 | Up to $218,000 | $202.90 | $0 |
| $109,001 to $137,000 | $218,001 to $274,000 | $284.10 | $14.50 |
| $137,001 to $171,000 | $274,001 to $342,000 | $405.80 | $37.50 |
| $171,001 to $205,000 | $342,001 to $410,000 | $527.50 | $60.40 |
| $205,001 to $499,999 | $410,001 to $749,999 | $649.20 | $83.30 |
| $500,000 and up | $750,000 and up | $689.90 | $91.00 |
Amounts are per person. The Part D extra is added to your drug plan's own premium. The 2027 brackets and premiums are usually announced in November and will be based on 2025 income.
How to lower or avoid IRMAA
- Watch the cliff. One dollar over a line costs the full surcharge for the year, so plan big income items with the brackets in mind.
- Spread out Roth conversions and large IRA withdrawals across years.
- Use pre-tax 401(k) or HSA contributions while still working to lower MAGI.
- Qualified charitable distributions from an IRA after age 70½ do not count as income.
- Appeal after a life-changing event. If you retired, lost income or a spouse died, file form SSA-44 to use more recent income.
How this calculator works
It compares your modified AGI with the 2026 IRMAA thresholds for your filing status and shows the Part B premium and the Part B and Part D surcharges for that tier. The yearly extra is the monthly surcharges times 12, times the number of people on Medicare. For married couples filing jointly, each spouse on Medicare pays the same tier.
